COVID-19 ALERT 05 | Crew Change Declaration Forms

TO: Our Valued Industry Partners,
In line with government proclamations to close borders and impose travel ban during COVID 19 heightened times, we have devised attached Declaration forms which we are proposing for all our crew to fill-up and sign:
- Declaration for new joiners – to be filled up and signed prior departure from Manila
- Declaration for onboard crew – to be filled up and signed by crew who are now onboard
These forms may serve as a contingency measure and address the obligations of parties under the contracts (POEA and CBA) during this extra ordinary times.
